Listed below are several forms that are used by the Guidance Department. They are grouped by grade. They are in PDF format, but by using "copy / paste" you should be able to complete them in whatever word processing platform you prefer.
This is an important form that helps your Guidance Counselor assist you with your college plans and also helps your Guidance Counselor formulate your letter of recommendation.
STUDENT RESPONSE FORMThis form is
one of the most important forms needed by your Guidance Counselor. It should be completed by students. We will work on this form in our Guidance Seminar classes, but often students need additional time to complete. This form is essential to the Guidance and Counseling staff as we help you navigate the college admissions process.

Teacher Rec Request FormThe Teacher Rec Request Form should be used to help facilitate the recommendation request process. Most colleges will require that students include a teacher recommendation as part of their application. Students are asked to complete and submit the Teacher Rec. Request to their teacher prior to the teacher's writing of the recommendation. The information provided on the form will assist the teacher in writing the recommendation.
updated NHS Teacher Rec Request[1].pdfStudent Appraisal FormThe Student Appraisal Form provides teachers the opportunity to briefly reflect on a student progress, potential, initiative, organization etc. Students are encouraged to request that at least two teachers complete the Student Appraisal Form (this should not include the teacher who will be writing the student's college recommendation). The completed form should be returned to the student's Guidance Counselor.
