Listed below are several forms that are used by the Guidance Department.  They are grouped by grade.  They are in PDF format, but buy using "copy / paste" you should be able to complete them in whatever word processing platform you prefer.

This is an important form that helps your Guidance Counselor assist you with your college plans and also helps your Guidance Counselor formulate your letter of recommendation.

STUDENT RESPONSE FORM
This form is one of the most important forms needed by your Guidance Counselor.  It should be completed by students.  We will work on this form in our Guidance Seminar classes, but often students need additional time to complete.  This form is essential to the Guidance and Counseling staff as we help you navigate the college admissions process.
